Veterans' mental health needs moved to the forefront

How does Bexar County go from the backwater to the forefront of mental health services for veterans?

In one fell swoop.

With last week's green light from District Attorney Susan Reed to develop a pretrial jail diversion program for certain combat-affected veterans, the county has a unique opportunity to lead the pack on mental health care.

In what some regard as a too-low-under-the-radar move, the Center for Health Care Services is developing a grant-funded program to offer post-conviction jail diversion and treatment to veterans with trauma-related mental health needs. According to Aaron Diaz, director of crisis and diversion services for CHCS, the state recognized Bexar County for its effective mental-health and substance-abuse jail diversion programs and sought it as a pilot for the veteran-focused services.

The grant, from the federal Substance Abuse and Mental Health Services Administration, is an annual renewable award of $394,000 to the CHCS. The program covers a five-year period from fiscal year 2010 to 2014 and would serve about 120 veterans annually.

Diaz said CHCS should be ready to start screening arrestees in about two months, and county officials are fine-tuning the adjudication process.

If the pattern seen in the county's drug and mental health courts is any indication, the combination of a pretrial veterans court programand post-conviction services targeting veterans holds great promise for addressing the root causes of mental illness among this group.

The recidivism rate among those who've been processed through one of the county's three drug courts is 8 percent. In the newer mental health court, in place for two years, recidivism rates among the 108 people who've been served is about 25 percent.

Not impressed? Consider that the recidivism rate for the total jail population is 80 percent.

Reflection on the trends in these courts is essential because there are parallels to the experiences of mentally ill veterans in the criminal justice system. An estimated 60 percent of incarcerated veterans have a substance dependency and nearly 30 percent have a serious mental illness. In most cases (70 percent), the incarcerated veterans are doing time for nonviolent crimes.

After months of lobbying Reed to drop her opposition to a pretrial jail diversion program for veterans who have a mental illness or traumatic brain injury tied to hazardous duty, now there is momentum. Last week, Reed's office agreed to be part of a hybrid pretrial and post-conviction program targeting veterans. The pretrial element will apply only to veterans acutely affected by hazardous duty who are accused of “low-level” misdemeanors.

Reed's approval “was imperative, and we're very happy that happened,” Diaz said.

There still are some valid fiscal concerns for county lawmakers going forward. The courts and adult probation department, for instance, won't share in the grant funding to support the veteran-focused efforts.

For now, though, there's an uncommon opportunity to make a difference in the lives of mentally ill veterans. Let's seize it.

Among the many in business attire who gathered Wednesday for a jubilant dedication of the new Haven for Hope campus, Richard Hartzog worked quietly in an old T-shirt, ferrying chairs back and forth across a crisp, serene courtyard.

Hartzog, 50, is homeless, exactly the sort of person for whom the $100 million transformation center was built. An addict of gambling and morphine, he's been volunteering at the burgeoning campus — sweeping, scrubbing bathrooms, washing windows — for the past month.

A resident of the SAMMinistries emergency shelter, he received an unexpected reward from NuStar Energy Chairman Bill Greehey, who put his arm around Hartzog during a break.

“You know what,” Greehey said, “I'll tell (SAMMinistries) the ones that have been working over here ought to be the first ones” to move in.

Greehey, who was the driving force behind making Haven for Hope a reality, later took to the podium to rejoice that opening day of the homeless campus finally had arrived.

“It's pretty amazing that in less than three years we've gone from a vision for Haven for Hope to a fully operational campus,” said Greehey, who's also chairman of Haven for Hope. “It takes some people three years to build a house.”

Greehey introduced a long list of elected officials, social service workers and other dignitaries who played roles in the development of the 37-acre campus, which will provide a range of services for nearly 1,400 homeless people, from showers, beds and meals to job training, counseling and education.

“Haven for Hope's primary mission is to provide caseworkers that will work with the homeless on transformation,” he said. “These folks are going to spend 40 hours a week going to class or working. I think we will see a success rate of more than 60 percent, meaning one year later, they will have a job, be living on their own and on a self-sustaining basis.”

The 940-bed campus on the western edge of downtown will open to men on April 26; women and families begin arriving in May. An outdoor courtyard will sleep another 400 people. Greehey said he expects the campus to be fully operational by June.

“This has truly been a community effort,” he said. “It's been a collective effort by the state, county, city and the private sector. We've raised almost $100 million in a very poor economy, and we only have $2 million more to go.”

He celebrated the facilities that already are up and running, including the $6.1 million Restoration Center, which provides detoxification and counseling to substance abusers and the mentally ill; and the medical, vision and dental clinic, which since opening in March 2009 has seen 15,000 patients and provided 53,000 treatments.

So far, representatives from more than 190 cities and 11 foreign countries have visited Haven for Hope, which has become a national model for a transformational homeless center, Greehey said.

“We're going to be saving lives,” he said. “We're not going to solve the homeless problem overnight. There will be problems and obstacles. But we are going to be saving lives.”

Former Mayor Phil Hardberger, who got the ball rolling in 2006 when he dedicated himself to fixing the homelessness problem and tapped Greehey and then-City Councilwoman Patti Radle to head a task force, didn't underplay the significance of the day's dedication.

“Today we witness history,” he said. “When we help one person, we help ourselves. We share a common destiny but not a common path to our destiny. We're going to help thousands of people change themselves, and in that help, we are enriched. In their transformation, we are transformed.”

Hardberger noted that no nation or city is exempt from caring for the homeless, and every religion teaches that the well-off are to care for the poor and needy.

“San Antonio is not a wealthy city, but we are rich in compassion,” he said. “What Haven for Hope is about is giving the homeless back their dignity — breaking the cycle of poverty, getting kids back in school, breaking addictions, getting people out from under the bridge and giving veterans back their right place in society.”

Mayor Julián Castro and County Judge Nelson Wolff also made remarks.

“These are steel and wood buildings, but underneath is love, compassion and faith,” said Castro, who was appointed to a committee in 2003 to find solutions for the city's homeless problem and for years has quietly joined volunteers in the annual Christmas Under the Bridge effort to feed and clothe homeless people.

Wolff, noting the county has provided $11 million for Haven for Hope, said the campus marks a turning point for the county to begin treating the homeless, rather than criminalizing them.

“To just stick these people in the Bexar County Jail is wrong, even criminal to do that,” he said.

As our nation continues to debate the merits of federal health care reform, Houstonians should realize that we have our own health care debate happening right here at home. It involves the people living with mental illness who cycle in and out of our publicly funded emergency rooms, jails and mental health crisis facilities with little hope of stabilization.

While a part of this problem involves a basic lack of access to mental health services, many of these individuals, called “frequent fliers” by law enforcement and medical personnel who have constant contact with them, have simply been receiving the wrong kinds of services. They are victims of our state's crisis-driven system, which provides temporary and limited emergency care for people experiencing a mental health crisis but often fails to provide ongoing community-based services that keep them stabilized and out of expensive acute-care facilities.

While this is a problem faced throughout our state, Houstonians should know that several of our local leaders at the county, city, state and federal levels have stepped up and taken leading roles in addressing this serious public health issue. These individuals, too numerous to name here, should be commended.

An example of this local leadership is the Houston Police Department's Chronic Consumer Stabilization Initiative, or CCSI. HPD's mental health unit, in collaboration with the Mental Health and Mental Retardation Authority of Harris County and the Houston Department of Health and Human Services, implemented the CCSI as a pilot program in February 2009. The program provides intensive case management for the 30 individuals with a serious and persistent mental illness who have the most frequent encounters with HPD, and its main goal is to divert these chronic individuals away from their routine and repetitive encounters with law enforcement, provide them with opportunities to lead a more stabilized life and reduce excessive calls for service to the 911 system.

HPD recently issued its final report of the six-month CCSI pilot. Involuntary commitments for the 30 people were reduced by 76.4 percent while offense reports and calls for service involving these 30 decreased by 67.3 percent at a total cost of less than $3,900 per person. In contrast, an average stay at the Harris County Psychiatric Center, which is about 10 days, costs about $3,525 per person. In addition, the number of hours HPD officers spent dealing with this particular group was reduced by almost one-third, meaning officers were better able to deal with other calls for service and criminal activity.

It does not take either a mental health or law enforcement expert to know that these results are impressive. While HPD has long been a leader on mental health issues with its Crisis Intervention Team and Crisis Intervention Response Team programs, the CCSI is different in that it is not a reactive program. CCSI prevents police encounters with people with serious mental illness in the first place, and it provides these people the services they need to have a real chance of leading stable and productive lives that include fewer expensive crisis episodes. This kind of proven and effective program is what many of us have been asking for here in Houston and throughout the state for quite some time.

The city of Houston deserves recognition for funding and implementing such an innovative program. Continuing and expanding the CCSI program would be an additional accomplishment that all Houstonians could be proud of in the future, and I encourage our local leaders to make this a priority for the city.

OBJECTIVE: Crisis intervention team (CIT) training provides police officers with knowledge and skills to improve their responses to individuals with mental illnesses. This study determined changes in knowledge, attitudes, and social distance related to schizophrenia among police officers after CIT training. METHODS: A survey was administered to 159 officers immediately before and after a 40-hour CIT training program in Georgia. Pre- and posttest data were gathered from surveys taken between December 2004 and July 2005. RESULTS: After the training, officers reported improved attitudes regarding aggressiveness among individuals with schizophrenia, became more supportive of treatment programs for schizophrenia, evidenced greater knowledge about schizophrenia, and reported less social distance toward individuals with schizophrenia. CONCLUSIONS: This study supports the hypothesis that an educational program for law enforcement officers may reduce stigmatizing attitudes toward persons with schizophrenia.

Title: The Wilco Way…Collaborative Emergency Mental Health Response

Abstract: This session will use three actual psychiatric emergency incidents to demonstrate the operation of our award winning Mobile Outreach Team (MOT). Each case will highlight different aspects of our collaborative approach – joint CIT and MOT response, field assessments, expediting psychiatric admissions, the use of technology, telemedicine, working with EMS and 911 dispatchers, facilitating respite care, and follow-up services. This multi-media presentation will include 911 tapes and a demonstration of the department’s web-based patient database. Administrative concerns such as flex funding, logistics, data collection, and multi-jurisdictional and interdepartmental collaboration will also be discussed. Over the past four years, the Williamson County Mobile Outreach Team and Williamson County Sheriff’s Office Crisis Intervention Team have saved our county government over four million dollars by diverting mentally ill persons from jail and hospital emergency rooms to clinically appropriate services. Learn how your department can achieve the same in this practical case driven session.

Presentor: Annie Burwell currently serves as the Mental Health Director for Williamson County and leads the nationally recognized Williamson County Mobile Outreach Team. This hardworking team provides emergency mental health services in conjunction with law enforcement, the local mental health authority, and EMS. Annie received her social work degree from the University of Texas at Austin. Her experience in the mental health field includes: psychiatric acute care with children; experiential therapy with adolescents, with an emphasis on backpacking and challenge courses; school based mental health services, and crisis intervention and management. Ms. Burwell has significant training in grief, loss, and trauma interventions, and is a certified hostage negotiator. She serves on many community boards including the Williamson County Commissioners’ Court Mental Health Task Force, and was a co-recipient of the 2002 Harry Gold Award, the 2008 Heroes in the Fight Award, and the 2009 Exemplary Program Award from the Texas Corrections Association.
